b. always singular irrespective of the subject.
c. No "should" is to be added.
Just to explain subjunctive means demand or command verb like 'requires', 'ordered'. Here is the format:
Demanding verb (such as requires, ordered, etc) + THAT + ROOT form verb or BE (simple tense form such as 'disclose' above)
