The B-School Application | IELTS vs TOEFL?

Hi

I have seen that many schools exempt international students who have completed graduate education in English. Can anyone please guide me to any list of schools that require IELTS/ TOEFL scores from ALL international students, and does not exempt anyone.

Also, could you please tell me whether IELTS can be a substitute for TOEFL in all these schools?

Finally, which IELTS test is to be taken: academic or general?
