exceptional can be used in two ways:

1. unusually good, e.g. we will have to do something exceptional to win.
2. very unusual, e.g. in a few exceptional cases the treatment had alarming side effects.

clearly, the exceptional case in option A falls into the second usage, and that's the reason Dmitry calls it "This is not an exception."
