1. Yes, that's always a possibility in question-adaptive tests like the GMAT. And that's why the new ESR is in some ways worse than the old one. The old one gave us some idea about question difficulty, and it also excluded experimental questions. The new one doesn't exclude experimental questions, and it has no information about question difficulty.
2. Good question, but as I mentioned earlier, it's very hard to link accuracy to score, especially on the new GMAT.
