General GMAT Questions and Strategies | Simple and basic error log format!

Hi,

I am new to the world of error logs. I am preparing for my GMAT retake and one of the things that I realised I was doing wrong was not taking a proper note on my weakness and not making a list of areas where I was going wrong, especially in quants.

I went through some of the error log formats on the forum and they seem too complicated with a lot of charts and fancy bar graphs et al.

Can someone please share a very basic and simple version of an error log for a beginner?
