Since it is in past tense, narrowed it down to B and C.

But my concern was if "followed" is a proper verb in past tense (as required) or an -ed modifer.

As per an article by e-gmat, if the subject is the doer of the action then it is a verb and not an -ed modifier.

Also, C is considered wrong due to usage of Past Perfect tense.
