I ended up missing question number 3. After going through the OA, I understood my mistake. I basically hesitate in going back to the passage for certain questions.
Do you jot down notes while reading the passage?

I actually tried that approach in which I took notes from the passage and guess what my accuracy was very poor while solving passages.
